This agreement outlines academic, professional, and ethical expectations for students in the Career Catalyst Work-Study program. By signing below, the student commits to aligning workplace performance with school standards, driving professional growth, and ensuring legal accountability.

Academic & Attendance Standards
I will maintain a minimum 2.5 GPA. If I am absent from school on any given day, I am legally prohibited from attending my work site that day. School is my first priority.
Professional Etiquette & Safety
I will adhere to all workplace guidelines, professional dress codes, safety protocols, and privacy/confidentiality rules of my host site as an ambassador of my high school.
Communication & Punctuality
I will report to my site on time. If I must be absent, I will contact BOTH my workplace supervisor and faculty advisor at least 2 hours before my scheduled shift starts.
Weekly Progress Logging
I will submit my verified timesheets and completed reflective journal assignments to my faculty advisor every Friday by 3:00 PM to receive credit.

Academic Acknowledgment: I understand that my work-study placement is an off-campus extension of the classroom. Failure to comply with school policies, work-study terms, or direct supervisor instructions will result in immediate removal from the placement and a failing grade for this term's work-study course.

EMPLOYER CONTRACT
This host site partnership contract establishes cooperative terms between the school's Work-Study program and the partner employer. The goal is to provide a structured, safe, and skill-aligned learning experience that builds professional agency in high school students.

Safe Work Environment & OSHA Guidelines
We agree to maintain a safe, clean, and harassment-free workplace. We will supply all required personal protective equipment (PPE) and deliver a formal safety orientation on the student’s first day of work.
Active Mentorship & Role-Specific Training
We will assign a designated site supervisor to provide ongoing direction, answer industry-related questions, explain tasks clearly, and model high professional standards.
Labor Law Compliance & Non-Displacement
We agree to adhere strictly to state and federal child labor laws regarding hours, hazardous tasks, and breaks. High school students will not displace or replace regular paid employees.
Bi-Weekly Evaluation & Feedback
We will verify student-reported hours and complete two brief progress feedback forms per semester, grading the student's punctuality, initiative, teamwork, and communication skills.
Day-One Safety & Onboarding Checklist
The following checklist must be completed by the Site Supervisor during the student’s first week of placement:

Liability & Notification Agreement: By entering this contract, the host site commits to treating the student as a career-learning trainee. In the event of workplace accidents, safety incidents, disciplinary problems, or prolonged unexplained student absences, the supervisor must notify the school coordinator within 24 hours.

This document grants permission for your high school student to leave school grounds to engage in an off-campus work-study placement. It clarifies the school’s academic expectations, transportation terms, and medical emergency guidelines.

Transportation Liability Acknowledgment
The school district does not provide transportation to work-study sites. Please check all transportation methods authorized for your student:

Student will use municipal bus, rail, or other public systems.

A family member or guardian will transport the student to/from the site.

Assumption of Risk & Release: I hereby release the school district, its board members, employees, and volunteers from all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature, occurring during transportation or while participating at the off-campus workplace site.
