Essential Facilitation Guardrails
Avoid Fixing Problems: The transitioning phase triggers intense anxiety. Facilitators should model sitting with uncertainty rather than jumping to offer immediate physical solutions or "back in my day" stories.
Validation First: Seniors feel heavy pressure to look figured-out. Normalize feeling completely split (e.g., ecstatic about freedom yet terrified of bills/social loss).
Pathway Equity: Actively monitor and correct any systemic bias in group language that portrays college as the "default" or "highest tier." Validate trade schools, careers, military, and gap years with equal weight.
Logistical Preparedness: Have printable worksheets ready before students arrive. Always lay out scenario cards or tactile assets prior to warm-ups.

PHASE 1 Identity & Emotional Grounding
FACILITATOR ACTIONS
Session 1
The Transition Landscape
30-35 Min
Focus: Developing safe group parameters, examining the dual nature of change, and mapping post-high school dreams versus fears.
Icebreaker (5 min)
The Baggage Claim
Students write one Hope and one Fear on blank sticky notes. Collect anonymously, mix up, post on the board, and read aloud to illustrate shared anxieties.
Skill Building (15 min)
Identity & Safety Planning
Discuss leaving the "senior bubble." Introduce the Identity Shift Concept (letting go of old high school labels to build an intentional post-graduation self).
Activity (10-15 min)
Landscape Mapping
Direct students to Reflection Journal: Page 1. Guide them through mapping current comforts vs. future unfamiliar terrain.
Suggested Script Hook
"For twelve years, your job has been 'student' inside a highly structured box. In just a few months, that box breaks. It's totally normal to feel excited for independence and terrified of the unknown at the exact same time. This group is our locker room to unpack it all."
Session 2
Mapping the Mind (CBT)
30-35 Min
Focus: Introducing the Cognitive Triangle and identifying patterns of automatic negative thoughts related to graduation pressure.
Icebreaker (5 min)
Physical Stress Scavenger
Ask: 'Where do you feel stress first?' (Jaw clenching, tight shoulders). Map these somatic responses on a board outline to emphasize body-mind link.
Skill Building (15 min)
The Cognitive Triangle
Teach: Thoughts drive Feelings drive Behaviors. Contrast Catastrophizing with objective Reality Reframing ("It will be a learning curve, but I've done hard things before").
Activity (10-15 min)
The Thought Deconstruction
Direct students to Reflection Journal: Page 2. Lead them to intercept an anxious future-focused thought and run it through the "Is it Fact or Opinion?" court trial.
Suggested Script Hook
"Your brain loves to protect you by telling terrifying ghost stories about the future. But just because a thought pops into your head doesn't mean it's a cold, hard fact. Today, we learn how to put our worst-case thoughts on trial."

PHASE 2 Actionable Skills & Support Nets
FACILITATOR ACTIONS
Session 3
Driving the Ship (EF & Planning)
30-35 Min
Focus: Managing massive lists of upcoming transitions without clinical paralysis through micro-stepping and Eisenhower task filtering.
Icebreaker (5 min)
The 24-Hour Auction
Give students "24 tokens" representing hours. Auction off items (sleep, studying, gaming, side-hustle, laundry). Notice who runs out of baseline self-care hours.
Skill Building (15 min)
Micro-Stepping & Eisenhower Matrix
Show how overwhelming goals cause paralysis. Demonstrate how to split massive actions into 15-minute micro-steps. Run a task sorting exercise on board.
Activity (10-15 min)
The Focus Funnel
Direct students to Reflection Journal: Page 3. Guide them to select one high-stress task and unpack it into 4 actionable micro-steps.
Suggested Script Hook
"When you have fifty transition tasks hanging over you, your brain simply freezes. You aren't lazy; you are overwhelmed. Today we learn how to carve massive mountain-sized goals into tiny pebble-sized steps that you can kick out of the way in 15 minutes."
Session 4
The Support Web (Relational Support)
30-35 Min
Focus: Mitigating the threat of loneliness by intentionally cataloging clinical, professional, and personal networks before leaving high school.
Icebreaker (5 min)
Web of Truths
Use a ball of yarn. Hand it to one student; they state an unvoiced social worry, hold a string end, and toss the ball to another. Reveal the physically tangled web of shared worry.
Skill Building (15 min)
Support Scaffolding
Differentiate between Functional support and Emotional support. Model "The Help Script": asking for clinical or academic support without shame.
Activity (10-15 min)
Mapping My Crew
Direct students to Reflection Journal: Page 4. Map their safe circles: immediate contacts, digital lifelines, and localized community services.
Suggested Script Hook
"Loneliness is the single biggest threat to fresh grads. We assume making friends is natural, but it takes intentional structure. Let's catalog your current lifelines and build a physical script to call in structural backup when things get heavy."

PHASE 3 Application & Transition Launch
FACILITATOR ACTIONS
Session 5
Scenario Showdown (Problem Solving)
30-35 Min
Focus: Active testing of cognitive and planning strategies via immersive, reality-based scenarios spanning multiple post-high school pathways.
Icebreaker (5 min)
What's the Verdict?
Read out an exaggerated post-grad disaster scenario (e.g., getting a flat tire on first day of work). Rate the panic response on a 1-10 scale.
Skill Building (15 min)
Adaptive Strategy Application
Divide the group into pathway-aligned teams. Hand out Scenario Cards (from Activities Pack). Dissect stressors into reframing and micro-steps.
Activity (10-15 min)
Response Blueprint
Direct students to Reflection Journal: Page 5. Have teams write out their structured crisis action plans and present to the circle.
Suggested Script Hook
"Life after graduation isn't perfect, and things will break. Your roommate will steal your milk, your boss will yell at you, or your train will be delayed. Let's stress-test our CBT and problem-solving tools in a safe space before you face these out in the wild."
Session 6
Launching Out (Closure & Pact)
30-35 Min
Focus: Solidifying key takeaways, mutual group appreciation, and formalizing individual commitment contracts for self-care.
Icebreaker (5 min)
Affirmation Hot Seat
Each student takes turns hearing the rest of the group point out one strength they demonstrated. Note these strengths on takeaway cards.
Skill Building (10 min)
Transition Celebration
Celebrate the completion of the cohort. Review core strategies: Cognitive Triangle, Focus Funnels, and Crew Maps.
Activity (15-20 min)
The Transition Pact
Direct students to Reflection Journal: Page 6. Students construct and sign their Transition Pact self-care contract.
Suggested Script Hook
"We've spent six weeks mapping this crossing. You're ready. This final contract is a commitment to yourself—your boundaries, your mental health, and your strategies. Hold onto it, read it when things feel shaky, and remember that you are built to survive the transition."

WEEK 1 Identity & Comfort Zones
The Transition Landscape
Leaving the structured bubble of high school is both exciting and terrifying. You are letting go of comfortable childhood roles to build your post-grad self.
Reflect & Share
"What is the single biggest comfort you are leaving behind? What is the biggest hope you are stepping toward?"
Leaving the Familiar
Structured schedule, parental supervision, daily interactions with long-term childhood friends.
Navigating the New
Independent scheduling, bill budgeting, self-driven study and shift hours, raw accountability.
Goal: Moving through discomfort intentionally Slide 2 of 7
WEEK 2 CBT & Coping Skills
Mapping the Mind
Stress doesn’t come from situations themselves. It comes from the stories your brain writes about those situations.
Weekly Skill
Catch negative automatic thoughts and put them on trial. Ask: Is this a fact, or is it an opinion?
The Cognitive Triangle
Thoughts
What you tell yourself about the future.
Feelings
The somatic emotions (anxiety, dread, panic).
Behaviors
What you do (avoiding, sleeping, scrolling).
Change your THOUGHTS to redirect your FEELINGS & BEHAVIORS!
Goal: Catch, challenge, and reframe negative thoughts Slide 3 of 7
WEEK 3 Executive Functioning
Driving the Ship
Massive transitions trigger massive task lists (financial aid, course sign-ups, job applications, equipment). Overwhelm leads to avoidance and brain-freeze.
Micro-Stepping Strategy
Break any goal down into a tiny step that takes under 15 minutes to complete.
1
Define the Target
Pick one major upcoming task causing you stress.
2
Isolate the Very First Move
Find the first physical step (e.g., opening a tab, sending 1 email, saving a PDF).
3
Block 15 Minutes
Set a physical timer. Execute only that first move, then walk away.
Goal: Conquer executive paralysis with tiny, tangible actions Slide 4 of 7

Mapping Your Senior Comfort Bubble
For twelve years, your environment has been highly organized. To navigate the crossing successfully, we must first map what comfort labels you are leaving behind and what unfamiliar spaces you are stepping toward.
01 / MY COMFORT ZONE LABELS
What are the familiar parts of your identity or school life that feel safest to you? (Examples: being a specific sport athlete, having your friend group's core spot, a set daily schedule, living in your family home).
02 / THE UNFAMILIAR PATHWAY
What pathway are you stepping into next? (College, career, trade school, military, gap year). What is the biggest, most intimidating differences or unknown factors about that path?

Putting Your Automatic Negative Thoughts on Trial
Our thoughts are often dramatic interpretations rather than raw facts. Use this court sheet to catch a highly anxious thought and dissect it.
1. The Anxious Automatic Thought
Write the catastrophizing thought you catch yourself repeating (e.g., "I'm going to fail out immediately" or "I'll never make new friends").
Evidence FOR the thought
What are the cold, physical facts supporting this thought? (No opinions or feelings allowed).
Evidence AGAINST the thought
What are past experiences, credentials, or actual facts that contradict this thought?
3. The Objective Reframed Verdict
Reconstruct the thought based 100% on the factual evidence. Make it a fair, realistic statement.

Transition Task Micro-Stepping
Massive goals trigger massive brain freeze. By breaking a mountain-sized task into minor 15-minute operational steps, you can conquer systemic avoidance.
A / THE LOOMING MOUNTAIN (Overwhelming Task)
Write down one major task that makes you freeze up whenever you think about it (e.g., "Submit financial aid forms," "Find roommate," "Register for trade exams").
B / PEBBLE-SIZED MICRO-STEPS (15 Minutes Max)
Break down that giant task into 4 physical, actionable steps. Each step must be so small that you can complete it in under 15 minutes with a phone or computer.
