6-Week Academic Recovery & Habit Intervention Guide
Cohort Framework 9th Grade On-Track
Why the 9th Grade On-Track Metric Matters
Research proves that high school freshmen who end the year "on track" (passing all core courses and earning at least 5 credits) are four times more likely to graduate than their off-track peers. Academic recovery during the first year is the single most critical leverage point for drop-out prevention.
6-Week Intervention Scope & Sequence
Weeks 1–2
Diagnosis & Alignment
Conduct comprehensive transcripts review, diagnostic interview, and align student-parent-mentor expectations.
Deliverable: Action Contract
Weeks 3–4
Habit Engineering
Implement systemic agenda tracking, active study protocols, and tackle "backlog" credit/assignment recovery.
Deliverable: Weekly Habit Log
Weeks 5–6
Stabilization & Exit
Transition ownership to the student, cement GPA-monitoring routines, and formulate independent study plans.
Deliverable: Exit Plan
Program Roles & Responsibilities
| Role | Key Responsibilities | Target Frequency |
|---|
| Program Mentor | Conducts check-ins, audits gradebooks, provides coaching, and coordinates recovery tasks. | Weekly (30 min meeting) |
| Classroom Teacher | Verifies assignment lists, accepts prioritized work, and communicates test retake opportunities. | Bi-Weekly status update |
| Parent / Guardian | Monitors school portal, logs homework hours at home, and rewards progress. | Weekly signature check-in |

The 30-Minute Check-In Protocol
1
Relationship & Mindset Check (5 Minutes)
Begin with an informal "high/low" check-in. Build trust by keeping initial questions focused on interest areas before diving into data. Ask: "What went well this week outside of classes? What felt heaviest?"
2
Systemic Grade & Portal Audit (10 Minutes)
Log in to the online school portal together. The student must read their grades aloud. Identify newly added missing homework items, upcoming exams, or failed quiz scores. Key Rule: Do not guess; rely entirely on verified portal data.
3
Habit Auditing & Notebook Check (5 Minutes)
Physically inspect the student's organizational tools. Review the class planner, note taking system, and backpack organization. Check for missing study resources or loose worksheets.
4
Targeted Weekly Action Plan (10 Minutes)
Co-create exactly three highly prioritized academic targets for the upcoming week (e.g., retake Math quiz during advisory, finish History outline, study flashcards for 15 minutes/day). Ensure each target is recorded on the Student Planner.
Intervention Response Blueprint
Observed Barrier: "Helplessness"
Behavior: Student complains they "don't understand" and refuses to start.
Response Strategy: Break assignments to micro-tasks. Practice first problem together, then let student try next.
Observed Barrier: "Avoidance"
Behavior: Student repeatedly forgets notes or lies about due dates/missing tasks.
Response Strategy: Implement weekly counselor-parent checks. Do not accept student word without grade portal verification.
Stakeholder Outreach Templates
Weekly Parent SMS Script:
"Hi [Name], this is [Mentor] from [School]. [Student] completed their check-in today! This week we are prioritizing passing the Math retake on Thurs. Please ask them to show you their study sheet tonight. Thanks!"
Teacher Alignment Email Draft:
"Hi [Teacher], I am supporting [Student] with Freshman academic recovery. They have identified your course as a priority to raise their grade. What is the most critical missing assignment they can submit this week to improve?"

Student Intake Interview (To be filled out during baseline meeting)
1. Why do you believe your grades in core courses dropped below passing?
Check all that apply: ☐ Attendance / Truancy ☐ Missed assignments ☐ High anxiety / Lack of focus ☐ Personal struggles
2. Where and when do you currently do homework or prepare for assessments?

Week 4–6 Adjustment & Graduation Plan:
Intervention Exit Verification
An individual exits the protocol upon satisfying two criteria: (1) Zero core subjects failed, and (2) A baseline cumulative or quarter grade average ≥ 2.0 GPA. If targets are unmet at week 6, administrative review will dictate counselor placement in Tier 3 alternative education plans.

INTERVENTION TOOLKIT
Evidence-Based Habits and Grade-Recovery Best Practices
Reference Toolkit Module 5
Executive Functioning Habits
- 1. The "Clean Slate" Backpack Drill Every Friday, the student dumps their backpack completely. Unused worksheets are filed chronologically, and trash is discarded. This reduces physical cognitive load and anxiety.
- 2. Micro-Task Scheduling Teach students not to write general statements like "Study History". Refine this to: "Read pages 14-16 and write down 3 key vocabulary definitions". Actionable prompts prevent mental blocks.
- 3. The 25-5 Study Method (Pomodoro) For students struggling with attention, enforce 25 minutes of high-focus study (no notifications, phone in backpack) followed by a strict 5-minute break. Repeat.
Academic Recovery Tactics
- 1. Prioritizing Grade "Leverage Points" Teach the student to check weight categories. An unfinished test retake worth 35% of the total grade always overrides several missing classwork grades worth 2%.
- 2. Self-Correction & Test Retakes Students should analyze their errors before a retake using a simple Error Log (e.g., Simple Calculation mistake vs. Conceptual mistake). This ensures active learning occurs.
- 3. Structured Peer Accountability Buddy system enrollment. Pair the off-track freshman with an on-track classmate inside the same course to share study guides, reminders, and coordinate study sessions.
Three-Tier Intervention Escalation Protocol
Tier 1: Preventive
Criteria: Grade averages hovering near passing (D/C border).
Action: Standard 1-on-1 weekly check-ins, routine grade logs.
Tier 2: Targeted
Criteria: Fails in 1–2 core classes. Excessive missing assignments.
Action: Daily check-ins, direct teacher communication, parent outreach.
Tier 3: Intensive
Criteria: Fails in 3+ classes, severe truancy, behavior triggers.
Action: Formal multi-disciplinary team meeting, IEP/504 review.
Administrative Program Compliance Sign-off
To complete this guide, verify that the initial parent contact has been made, the student goal sheet is completed, and both physical student workbooks have been distributed. Keep this guide filed under student intervention records.

Discovering "My Why"
High school is about building the future YOU want. Graduation isn't just about credits; it is about keeping your doors open for sports, careers, technical fields, and independence.
What are two positive things that will happen when you graduate high school?
My Academic Habits Self-Audit
Be 100% honest with yourself. Rate where you are right now (1 = Not started, 5 = Mastered):
I log into my grade portal at least once a week to review missing work.
1 2 3 4 5
I have a dedicated study space at home with zero phone/screen distractions.
1 2 3 4 5
I write down my upcoming tests and homework due dates in a planner/agenda.
1 2 3 4 5
I ask my teachers for help or quiz retakes when my grades are below a C.

My 6-Week Turnaround Targets
Academic Goal (GPA / Classes Passed)

DAILY HABIT BLUEPRINT
Unlocking the Routines That Build Top Students
Skill Builder Module 2
The Habit Loop Strategy: Cue → Routine → Reward
Habits form when we attach them to triggers that already exist. If you struggle to study at home, design a trigger: "As soon as I walk through my front door, I will immediately put my phone in the kitchen drawer and do 20 minutes of study at my desk before I get a snack."
My Weekly Habit Builder Ledger
