IEP Diagnostic Protocol Progress Monitoring Kit
Multisyllabic Decoding & Fluency Probes
5-Form Standardized Assessment Battery for Decoding & Oral Reading Fluency Objectives
Form: Progress Battery A–E
Target Range: Grades 3–5 / IEP Level
Student Name ______________________
Case Manager / Examiner ______________________
Baseline WCPM / Date ____ WCPM (___/___/___)
IEP Goal Target ≥ 85 WCPM / ≥ 95% Acc
Goal 1: Multisyllabic Decoding
Given grade-level multisyllabic text, student will break 2-, 3-, and 4-syllable words into decodable syllable chunks and affixes with at least 85% accuracy across 4 consecutive probes.
Goal 2: Accuracy & Fluent Expression
Given an unfamiliar passage, student will read aloud at a minimum rate of 80–95 WCPM with at least 95% accuracy and a rating of 3 or higher on the NAEP Fluency Rubric.
Standardized Administration & Scoring Rules
1. Examiner Script
"Please read this story out loud. If you reach a long word, do your best to chunk it into parts and keep reading. Begin when ready."
2. Timing & Tracking
Start 60-second timer on word one. Mark uncorrected errors with a slash (/). Bracket ( ] ) last word read at 60s. Let student finish passage for decoding diagnostic.
3. Error Criteria
Mispronunciations, omissions, syllable truncations, and hesitations >3 sec count as errors. Self-corrections within 3 seconds are counted as correct.
Six Syllable Types Reference
Closed (C): in-spect, fan-tas-tic
Open (O): ro-bot, lo-cate
VCe (V-e): ex-plore, con-clude
Vowel Team (VT): re-veal, mai-ntain
R-Controlled (VR): per-form, har-bor
Consonant-le (Cle): ob-sta-cle, gen-tle
NAEP Qualitative Fluency Rubric
4 - Expressive: Fluent phrasing, expressive inflection, follows syntax.
3 - Conversational: 3–4 word phrasing, appropriate pauses, minor halting.
2 - Uneven: Choppy two-word units, awkward rhythm, limited expression.
1 - Disfluent: Word-by-word reading, labored pauses, no expressive tone.

The brave explorers began their dif·fi·cult ex·pe·di·tion up the steep mountain. [11] Heavy backpacks contained important equipment and pro·tec·tive blankets. [20] Suddenly, freezing tem·per·a·tures transformed the rocky pathway into slippery ice. [30] Their navigator observed the sur·round·ing horizon with powerful binoculars. [39] She signaled everyone to proceed carefully across the narrow boundary. [49] Fortunately, their de·ter·mi·na·tion kept the entire group confident and united. [59] By afternoon, they discovered an incredible shelter nestled inside the canyon. [69] The scientists immediately unpacked their instruments to conduct their in·ves·ti·ga·tion. [79] This remarkable ac·com·plish·ment represented months of dedicated preparation. [88]
