| 1. Arrival & Workspace | | | | |
Entry demeanor, phone zone, bag stowing, gear unboxed within 3-4 mins.
Score: [ ] / 4
| Enters quietly without delay; phone stowed immediately; backpack hung/under desk; needed materials out in under 2 minutes completely unprompted. | Enters calmly; stows phone and bag; unpacks materials within 3–4 minute target window following classroom visual checklist cue. | Wanders or delays unpacking; requires 2+ teacher reminders to stow bag, put phone away, or retrieve basic supplies. | Resistant to entering calmly; refuses phone check or seating; materials disorganized or absent; stalls past 6 minutes. |
|
2. Digital Initiation
Closing non-work tabs, opening LMS/Classroom, beginning sprint task.
Score: [ ] / 4
| Closes off-task tabs instantly; logs into assigned portal; writes priority sprint goal and initiates work before the transition timer rings. | Opens required academic sites (Google Classroom, Infinite Campus); has sprint goal written; begins work with minimal teacher check. | Keeps off-task tabs/games open; slow to log in; needs repeated redirection to identify sprint task and begin working. | Refuses digital login or actively hides gaming/social sites; unable or unwilling to begin academic task without 1-on-1 facilitation. |
|
3. Regulation & Help
Accurate self-rating, tool activation, appropriate help-seeking signaling.
Score: [ ] / 4
| Accurately identifies internal state; independently deploys coping strategy (headphones, timer); uses subtle desk cue for teacher support. | Completes readiness check; selects a viable reset strategy; requests teacher assistance appropriately (sticky note, cue card, hand signal). | Skips regulation check; shuts down or calls out loudly when stuck instead of utilizing designated help-seeking protocols. | Dysregulated behavior; resists coping tools; escalates frustration verbally or physically; refuses teacher support efforts. |
|
4. Community Focus
Voice level 0-1, body boundaries, zero attention-seeking or peer interruption.
