Clinical Progress Statements (Ready to Transcribe)
Option A: Comprehensive IEP Quarterly Progress Statement Standard IEP Format
During this reporting period, data collection was conducted across two direct observation sessions. The student demonstrated emerging functional self-advocacy skills, communicating requests and assistance using words and word approximations with direct verbal and visual models. Spontaneous or independent communication was not yet observed during these trials. The student continues to benefit from adult modeling to initiate requests and express basic needs. Self-advocacy and functional communication will continue to be targeted in upcoming sessions to foster independence.
Option B: Concise Clinical Log / Electronic Health Record (EHR) Note Brief Log
Across 2 data collection sessions, student required direct clinician models to communicate wants, needs, and requests using words/word approximations (0% independent). Spontaneous self-advocacy and protests were not observed. Goal will continue to be addressed during the upcoming reporting period with faded prompting.

Intervention Plan (Next Period)
- Communication Temptations: Sabotage routine activities to evoke natural requests.
