1. Present Level of Academic Achievement & Functional Performance (PLAAFP)
Current Academic Functioning: When presented with grade-level literary fiction passages (Lexile 520L–680L), the student demonstrates the ability to identify explicit details when directly queried but exhibits significant difficulty synthesizing story grammar components into a cohesive narrative framework. The student consistently confuses the central narrative problem with isolated minor conflicts and rarely connects the character's resolution back to the initial conflict.
Impact on General Education Curriculum: Difficulty recognizing structural story elements inhibits the student from making textual inferences, participating in guided reading discussions, answering higher-order comprehension questions, and producing organized written responses about narrative text.
Quantified Baseline Data: In 4 baseline probes using 3rd-grade fiction passages without scaffolding, student achieved an average overall accuracy of 46% across all story elements: Characters: 75%, Setting (Time & Place): 60%, Key Plot Events (Seq): 40%, Central Problem: 30%, and Resolution/Solution: 25%.
2. Measurable Annual SMART Goal
Annual Goal By the end of the IEP cycle (September 2027), when given a grade-level literary passage (read independently or with approved text-to-speech accommodation) and a structured graphic organizer, the student will identify and explain all five key story elements—including major characters, setting (time and place), sequential key events (beginning, middle, end), central problem, and resolution/solution—demonstrating at least 80% accuracy across all 5 components across 3 consecutive weekly progress monitoring trials, as measured by standardized rubrics, work samples, and teacher records.
3. Scaffolded Short-Term Objectives & Benchmarks
Benchmark 1 (Quarter 1) Target: 80%
Given a short literary passage and a 2-part chart, student will correctly identify the main characters and setting (time & place) with citing 1 piece of text evidence in 4 out of 5 consecutive probes.
Benchmark 2 (Quarter 2) Target: 80%
Given a literary passage, student will accurately sequence and summarize at least 3 key plot events (beginning, middle, end) using temporal transition stems in 4 out of 5 consecutive trials.
Benchmark 3 (Quarter 3) Target: 80%
Given a literary text, student will accurately articulate the central problem and how the characters arrive at the solution, distinguishing from minor events, in 4 out of 5 probes.
Benchmark 4 (Quarter 4) Target: 80%
Given an unfamiliar grade-level narrative text, student will complete a 5-element story map analyzing all 5 story elements collectively with 80%+ accuracy across 3 consecutive sessions.

Visual & Graphic Support Color-coded 5-finger retell charts, visual anchor charts for problem vs. solution.
Instructional Scaffolding Pre-highlighted vocabulary, paragraph chunking, explicit modeled "think-alouds".

Progress Monitoring Decision Rules (Data-Based Decision Making)
Positive Progress (≥ 80% on 3 Probes): Advance student to independent identification without graphic organizer or introduce more complex narrative structures (e.g., multiple problems/flashbacks).
Insufficient Progress (< 60% across 3 Probes): Implement targeted reteaching on lagging elements (e.g., Problem/Solution pairing), increase visual color coding, and verify Lexile accessibility.
