Fact Error Pattern Diagnostic Guide
Tiered Intervention Triggers
Foundational Insecurity (0s, 1s, 2s, 5s, 10s)
Errors indicate lack of conceptual basis or skip-counting breakdown. Action: Number lines, array building, concrete dot grouping.
Derived Fact Hesitation (3s, 4s, 9s)
Slow response time (> 3 sec) indicates manual counting. Action: Teach "Double-Double" for 4s, "Minus 1 set from 10" for 9s.
Hard Combination Breakdown (6, 7, 8 & 12)
Frequent errors on 7×8, 6×7, 8×9, 12×7. Action: Daily flash sprints on 10 toughest combinations; distributive decomposition.

Scoring & Administration Guide
Standardized Protocol
Standardized Teacher Administration Script
Say verbatim to students: "Today we are doing a quick multiplication check. When I say begin, turn your page over and start with the first problem in the top row. Work across each row from left to right. Do not skip problems. If you come to one you really do not know, give your best estimate or move to the next. You will have exactly 2 minutes. Try your best. Pencils up... Ready... Begin!"
At exactly 2:00: "Stop. Put your pencils down immediately." (Have students circle their final attempted item with a colored pencil).
Scoring Conventions
- ✓ Count As Correct: Accurate products written in legible numerals; reversed digits that clearly indicate the target value (e.g. reversed '3').
- ✗ Count As Incorrect: Blank boxes, uncompleted items before the final attempt line, illegible digits, or wrong products.
- ℹ Skipped Problems: Mark as incorrect. If a student skips an entire row, remind them during debrief to proceed sequentially.
Metric Computation
Facts Per Minute (FPM):
\(\text{FPM} = \frac{\text{Correct Facts Scored}}{\text{Time in Minutes (e.g., 2)}}\)
Accuracy Percentage:
\(\text{Accuracy} = \left(\frac{\text{Total Correct}}{\text{Total Items Attempted}}\right) \times 100\)
MTSS Multi-Tiered Decision Framework (Grade 3-6 Benchmark)
Tier 1: Benchmark ≥ 30 FPM
Accuracy ≥ 90%. Automatic retrieval without observable counting lag.
Routine: Bi-weekly maintenance probes; apply to multi-digit multiplication & division.
Tier 2: Strategic 18-29 FPM
Accuracy 75-89%. Uses derived strategies slowly (e.g. finger skip counting).
Intervention: 10 min small group 3×/week. Focus on "Double-Double" & decomposition.
Tier 3: Intensive < 18 FPM
Accuracy < 75%. Lacks conceptual connection or retrieval automaticity.
Intervention: Daily 15 min 1-on-1 / Cover-Copy-Compare. Narrow to 2-3 new facts per week.

Evidence-Based Fluency Interventions
1. Cover-Copy-Compare
Student looks at fact with answer, covers it, writes from memory, uncovers to verify immediately. If error occurs, repeats 3 times.
2. Incremental Rehearsal
Flashcard stack containing 9 known facts and 1 unknown target fact. Keeps success rate at 90% while building rapid automatic retrieval.
3. Distributive Break-Apart
Deconstruct tough facts using 5s and 2s: e.g., \(7 \times 8 = (5 \times 8) + (2 \times 8) = 40 + 16 = 56\). Transition to direct recall.
